LifeMap Solutions, [Inc.] (“LifeMap Solutions,” “we,” or “our”) takes your privacy seriously. We know that you care about how your personal information is used and safeguarded, and we are committed to handling your information in a secure and responsible manner. Please read this Privacy Policy to learn how we will handle your personal information. By visiting or using our websites at www.lifemap-solutions.com (“Websites”), downloadable software that we make available to you (“Software”), and all services, data, and materials accessed via the Websites or Software (“Services,” and the Websites, Software, and Services are collectively “Properties”), you acknowledge and accept the practices and policies outlined here. This Privacy Policy applies to the Properties. This Privacy Policy does not apply to the practices of other companies or to websites, software, and web services that we do not control.
By browsing, creating an account for, accessing, downloading or using our Properties, you acknowledge you have read and understood this Privacy Policy and consent to our collection, use and disclosure of your information and data, including Personally Identifying Information (defined below) in accordance with this Privacy Policy, consistent with applicable state and federal laws and regulations. 2. Use of Information.
LifeMap Solutions receives, gathers, manages, analyzes, and shares information. We do so in order to operate and maintain our Properties and to do business with users and other customers, and business partners. Such information includes information about you that can be used to contact or identify you (collectively, “Personally Identifying Information”). Such information also includes Personally Identifying Information that relates to your past, present, or future physical attributes, physical or mental health, fitness, or provision of health care to you (collectively, “Personal Health Information”). Such information also includes information from which contact or identity information about you has been removed (“De-Identified Data”).
For example, we may receive, gather, manage, analyze, and share the following:
- Information collected automatically such as browser type, location, language preference, referring site, and the date and time of use of our Properties;
- Information that users of Properties provide to us;
- Information concerning the use of our Properties by users;
- Information about users of our Properties that we receive from business partners; and
- Information that a user of our Properties has made public.
Also by way of example, LifeMap Solutions may receive, gather, manage, analyze, and share information in order to correspond with users of our Properties. LifeMap Solutions may use information to verify the identity of users, to check user qualifications, and to follow up with transactions initiated on our Properties. LifeMap Solutions may use contact information to inform users of any changes to Properties or to send users additional information about LifeMap Solutions and its products and services.
With your consent, we may share De-Identified Data with customers, business partners, health researchers, and other parties.
LifeMap Solutions may, or may not, control the use of information by customers, business partners, health researchers and other parties with whom it shares any information.
3. Collection of Personally Identifying Information.
We collect and use the Personally Identifying Information that you choose to provide to us when you use our Properties. We also collect and use Personally Identifying Information automatically when you use our Properties, for example, through technical means such as browser cookies. This information may include, for example, your name, phone number, credit card number, email address, and home address. This information may include Personal Health Information that you provide to us when you use our Properties or that we obtain from third parties. You may at any time refuse to provide us with any such information, but your refusal may result in your not being permitted or able to use one or more of our Properties.
4. Sharing of Personally Identifying Information.
We will not rent, barter, share, exchange, or sell Personally Identifying Information to individuals or companies except we may share such information as reasonably needed to develop or operate the Properties or under the following circumstances.
a. Partner Enrollment. We may share your Personally Identifying Information with a Partner if you were enrolled in the Services or Properties through that Partner. A Partner may be a health care provider, a health insurance provider, health care system, or other trusted group. For example, we may share Personally Identifying Information with your health care team or health care provider if you were enrolled in our Properties through that health care team or health care provider.
b. Your Consent. When you use one of our Properties we may ask for and you may give us consent to share Personally Identifying Information with one or more third parties. For example, we may ask you to provide consent to our sharing Personally Identifying Information with your extended care team or health insurance provider. We may ask you to provide consent to our delivering Personally Identifying Information to a third party for use in a clinical trial study.
c. Your Healthcare Provider. Healthcare providers such as doctors and therapists sometimes arrange for the delivery of our Properties to patients as a part of the care they provide. We may share Personally Identifying Information with such a health provider who has arranged for such delivery to you. These providers will protect, maintain, and use such information according to their own privacy policy and practices.
d. Agents and Affiliated Parties. We may share Personally Identifying Information with our agents and affiliated companies who help us develop or operate our Properties.
e. Operational Partners. We may share Personally Identifying Information with third parties who help us develop or operate our Properties. These third parties may include, for example, companies that host our Services on computer servers or that help us develop the software code or ideas that underlie our Services.
f. Public Information. Our Properties may permit users to share Personally Identifying Information about themselves and/or to make it public, for example, through a forum, a bulletin board or other electronic delivery. LifeMap Solutions may disseminate this information. LifeMap Solutions cannot and does not control how this divulged information is used and further disseminated.
g. Good Cause. We may release Personally Identifying Information about you when we believe in good faith that release is necessary to comply with the law; to enforce or apply our conditions of use and other agreements; or to protect the rights, property, or safety of LifeMap Solutions, our employees, our users, or others. This includes exchanging information with other companies and organizations for fraud protection and credit risk reduction.
h. Promotional Offers. We will not share, lease, or sell your personal information to other companies for their marketing purposes, but we may send you offers that include promotion of products of other businesses. If you do not wish to receive these offers, please send an email with your request to [email protected], and we will process your request within a reasonable time. Please note that you may receive additional offers while your request is being processed.
5. Cross-Border Deliveries.
The facilities of LifeMap Solutions and its agents, affiliates, and business and development partners are located around the world. As a result, information provided to us by a user may be delivered to, and processed in, a country that mandates a lesser level of data protection than does the user’s home country.
6. Security.
We employ security measures designed to prevent the loss, misuse, and alteration of information, but we make no assurances about our ability to prevent any such loss, misuse, or alteration.
7. Other Sites and Services.
Our Properties may link to websites and services offered by various parties who maintain separate privacy policies. We do not control such parties or the terms upon which you deal with such parties. Dealings with any such party are between you and such party. We suggest that you read and understand the privacy policy of each party with whom you engage.
8. Children.
Children under the age of 13 may not use our Properties. If it comes to our attention that we have collected Personally Identifying Information from any person under the age of 13, we will endeavor to delete this information as soon as possible.
9. Access to Information.
Users may request access, correction, or deletion of Personally Identifying Information as required by law. LifeMap Solutions will seek to comply with any bona fide request. Requests may be made to LifeMap Solutions at the email address mentioned above.
10. Cookies and Web Beacons.
A cookie is a string of information that a website stores on a visitor’s device and that the visitor’s browser provides to a website each time the visitor returns. We use cookies to help us identify and track visitors, their usage, and their access preferences. A user who does not wish to have cookies placed on his or her device should set his or her browser to refuse cookies before using our Properties, with the drawback that certain features of our Properties may not function properly without the aid of cookies. This Privacy Policy does not cover cookies set by any third parties. A web beacon is a small (often invisible) graphic image or other web programming code embedded in a web page or email. Web beacons or similar technologies may be used by LifeMap Solutions for a number of purposes, including to count visitors to our Properties, to monitor how users navigate our services, and to count how many sent email messages are actually opened.
11. Business Transfers.
If LifeMap Solutions is acquired or sells all or a portion of its assets to another company, or declares bankruptcy, it is likely that information held by LifeMap Solutions will be transferred away from LifeMap Solutions. Nothing in the policy is meant to forbid such a transfer.
12. Change of Policy.
LifeMap Solutions may change its Privacy Policy from time to time. Your use of a Property after a change means that you accept the changed policy. Users should check this page to review any changes. LifeMap Solutions will endeavor to notify all registered users of its Properties of changes to this policy.
